Management of iatrogenic accidents Dr. Yoshi Terauchi (D.D.S., PhD, Tokyo, Japan) Although iatrogenic accidents cannot be prevented with absolute certainty, their occurrences can be greatly reduced with the aid of the "Three Ts": training, technique, and technology. At the same time, those accidents can be properly managed with the same three Ts without creating further complications once they have occurred. The technology in this regard includes the dental operating microscope (DOM) together with cone-beam computed tomography (CBCT), and both rotary and ultrasonic instruments which can be precisely used with accuracy based on the treatment plan made following the diagnosis. Of the iatrogenic accidents, ledge formation, perforation, radicular extrusion of root canal filling materials, and instrument separation are most frequently encountered in retreatment cases.
